Follow the tutorials.
When you create a project you need to chose one of the main modules (that would correspond the most to your game), then modify as needed.
The "empty" module is totally empty, no physics, no rendering, ... this module is meant for developers who want to build a new module from scratch.
You're not suppose to use it, unless you know what you're doing ;)

There is no such thing.
Follow all the tutorials from the official website, you'll learn how all them work, how to set up...
And dig in the code, analyse the scripts, how they're all set, how they work... and you might be able to make you own:

Update: in fact, just before the release of the 4.5... Joe released a tutorial series (the NESmaker Summer Camp tutorials) to show to modify a module to make your own. But that tutorials have been removed.
